Position Summary:


ChesPenn Health Services is seeking a Community Health Worker with a dedication to serving ChesPenn’s patients and the surrounding community. The Community Health Worker (CHW) serves as a bridge between the health center and patient and his/her family with the goal of improving health outcomes. The CHW leverages his/her knowledge of the local community and the health and human services resources in the area to educate and empower patients and their families to understand and manage their health needs.



  • High school diploma or equivalent
  • Some experience in health care preferred
  • Training in a CHW course preferred
  • Current COVID-19 and flu vaccines required


Knowledge, Skills, and Abilities


  • Current or former resident of the service community strongly preferred; experience in community/outpatient settings a plus
  • Genuine passion and desire to enter a helping profession in health care or social services
  • Completion of a Community Health Worker curriculum preferred
  • Bilingual/bicultural fluency preferred
  • Current COVID-19 vaccine required


What You’ll Do

  • Help patients develop health management plans and goals
  • Coach and coordinate patients in the management of their chronic health conditions and self-care
  • Establish positive, supportive relationships with participants and provide feedback
  • Facilitate communication and client empowerment in interactions with health care/social service systems
  • Facilitate communication and coordinate services between providers
  • Work effectively with stakeholders (patients, staff, partner agency representatives, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
  • Continuously expand knowledge and understanding of community resources, services and programs provided
  • Identify and apply appropriate role definition and boundaries
