Position Summary

ChesPenn Health Services is seeking a dynamic, people-focused social worker who will help our patients overcome socioeconomic barriers to achieving their health and wellness goals.
This is an opportunity to join a dedicated team of health professionals working to create a culture of health and wellness among residents of the greater Coatesville area.

 

Qualifications


  • Associate degree in Social Services, Bachelor's Degree a plus
  • Two (2) or more years in social services or related field
  • Current COVID-19 and flu vaccines required

 

Knowledge, Skills and Abilities:

 

  • Knowledge of community resources relevant to social determinants of health
  • Strong verbal communication skills, including excellent active listening skills
  • Excellent written communication skills
  • Ability to work harmoniously as part of a health care team
  • Ability to meet approved minimal physical and medical standards
  • Fluency in Spanish is a plus

 

What You’ll Do

Patient Support:

 

  • Provide individual and family supportive counseling and referral to help improve personal and social functioning and resolve emotional and psychological problems.
  • Provide advocacy assistance in obtaining necessary services.
  • Partner with the Community Health Educator and other agency representatives to identify and work to address unmet community needs.
  • Partner with community agencies to facilitate mutual referrals.
  • Provide documentation in a timely manner according to a department standard.
  • Model skillful, supportive, strengths-based interactions with patients and coworkers
  • Maintain confidentiality of information about patients and their families.
  • Perform other duties as directed by supervisor.

 

     Clerical:

 

  • Document all patient contacts, case updates, and referrals according to establish protocols and standards.
  •  Maintain statistical data necessary for reporting requirements.
  • Track appointment compliance for care-managed patients who have missed appointments, and document outcomes.
  • Prepare reports and audits requested by the supervisor.
